- Brownstone Event Registration: Please use this form to request alcohol-free events, philanthropy/service events, brotherhood/sisterhood/members-only events, mixers, and invites that will take place in any brownstone (University or Alumni owned).
- Recruitment Registration: Please use this form to register your chapter's recruitment events including interviews, bid days and initiations.
- New Member Discontinuation: Please use this form if you have a current new member who is choosing or has been chosen to be removed from joining the chapter.
- Multicultural Workshop Request: Please use this form to request a workshop from the FSL office on diversity/multiculturalism.
- FSL A/V Equipment Request: Please use this form to request to use FSL's designiated audio/visual equipment.
